Home health care allows individuals to receive care in the comfort of their own homes, promoting independence and enhancing quality of life while managing their health conditions effectively.
Home health care can be a highly beneficial alternative to inpatient care at a hospital or medical facility if you or a loved one recently had surgery, is undergoing cancer treatment, has been diagnosed with a serious medical condition, is recovering from a serious injury, or is otherwise in need of professional care. The goal of home healthcare is to treat an illness or injury and help you recover, regain your independence and become as self‑sufficient as possible.
The number of home healthcare visits is tailored to your individual needs, based on your physician's orders, Medicare regulations, and private insurance approval.
One of the main requirements for patients to be eligible for receiving home health services is that they are "homebound." Homebound means you have an inability to leave your home and, therefore, leaving home requires a considerable and taxing effort. Occasional absences from the home are permitted as long as they are of short duration. For example, doctor appointments, family reunions, funerals, religious services, or graduations will not disqualify you from home health services as long as they do not indicate you have the ability to obtain services in a setting other than your home.
After your physician makes the referral, an Aspire clinician will reach out to schedule your first appointment. Our clinicians will collaborate with your physician to create a personalized care plan and keep them informed of any updates or changes in your condition. Any modifications to your care plan will be made in consultation with your physician.
